2021 Community/Stakeholders Surveys

Sandhills Center values feedback from our members and their families, providers, community partners and other stakeholders.

As in previous years, we are once again requesting your assistance in completing the yearly GAPs Analysis Community and Stakeholders Surveys. Please note that all participant responses are anonymous. We are excited to receive your input so we can continue to work together to identity strategies and address any identified gaps.
