Sandhills Center requests stakeholder feedback
Sandhills Center is asking for assistance in completing the annual Community Needs Assessment Stakeholders’ Surveys.
There are two surveys — one for consumers, and one for providers and other stakeholders. Those who take a survey will have an opportunity to tell Sandhills Center what gaps in services may exist so that strategies can be developed to address them. Surveys can be taken online using personal computers, tablet devices or smartphones. Those who would like a paper survey may obtain one by calling Sandhills Center at 1-800-256-2452.
Typically, the surveys take five minutes or less to complete. All respondents will remain anonymous.

- For consumers (individuals diagnosed with/treated for a mental health challenge, a substance use disorder, or an intellectual/developmental disability, and/or their family members) — please click here.
